Every time your team plays a game you need to fill out the game card, with a list of
all players including their jersey number and their CYSA registration number.
To make your life easier you can fill out this form here and print the resulting page, cut it up
(you get 3 lists per page) and paste the list onto the game card (don't forget to take
some tape with you).
The second page of the generated PDF file contains a game card you can use
if you are the home team.
Note that we do not offer a way to store the game card info on our system yet, so you should
print out the form 2 or 3 times to get enough lists for one season, or at the very least store
the generated PDF file on your computer.
