League Forms

• Zero Tolerance Form - to be signed by all parents and players and given to their coach.
• Registration form for players - please read the instructions for Fall registrations or the the instructions for Spring registrations before filling out this form.
• Class 1/3 Coaches application form - all coaches need to fill out this form once a year.
• Coaches registration form - all coaches (comp and rec) need to fill out this form once a year. You can fill it out on the computer, but you need to print it 4 times, sign all 4 copies and send them to the registrar.
• Game Card List - required for every game your team will play.
• Referee sign-up form - Referees that want to work for the league need to fill out this form.
• Volunteer Donation/Reimbursement Form.
• Uniform Reimbursement Form - $20/player/year for comp teams, to be submitted by Dec 15.
