[SSJ-Coaches] uniform reimbursement request due by 12/15/08

This is a friendly reminder that it is time to send the uniform 
reimbursement form if you haven't done so already. 
You can download the form on our league's web site (ssjysl.org/forms).
With the completed form I need also a copy of the team registration for 
the fall season, and a  *color* picture of the team wearing our uniform.
