Dear Coaches,

This is a friendly reminder that it is time to mail in you uniform reimbursement request.
The deadline is Dec. 15.

I need:
- filled out reimbursement form (http://www.ssjysl.org/forms_uref.shtml)
- color picture of team wearing the correct uniform
- copy of golden rod (official team registration form)
