Fees:	Referee license fee is $45.00 payable to CNRA and $25.00 for books/class payable to SSJYSL
Where: Time:	Los Paseo Cabana Club 7047 Via Ramada Drive, San Jose, CA 95139. 6:00pm to 9:00pm each night. You MUST attend all the dates above.
Registration:	Class size is limited to 30 students. Pre-Registration is required at www.d2sra.org/train/clinicschedule.   Walk-in if space available.

How to Register:	You must register at http://www.d2sra.org/train/clinicschedule and then print the registration form and mail it along with One check($45.00) payable to CNRA and one check ($25.00) payable to SSJYSL. Drop off in secure mailbox or mail to Christine Alves 282 Moraga Way, San Jose, CA 95119.  Confirmation via email once the checks are received.
