Referee Signup

If you are interested in refereeing for the South San Jose Youth Soccer League please fill out the Referee Signup Form.  Mail a copy of your 2008 Referee license and a W9 with this form to the address below. Once the documents are received, you will be sent an email from THE ARBITER to self assign games.

Referee Evaluation Form

In the event that you need to evaluate a referee, please use the League Referee Evaluation form.

This form is available in pdf format, which requires Adobe Acrobat Reader. Print the form and return it to the Referee Director at:

Send off (Red card) Form

Please print the send-off and fill it out for every red card and send-offs.

Red Card Verify Form

The Red Card Verify Form can be downloaded from the web page (requires OpenOffice or Word).

W-9 Form

New licensed referees to SSJYSL who wish to be paid must have a W-9 form on file with the SSJYSL Treasurer. Download a blank W-9 and send the completed form, including a copy of your 2008 referee license, to:
